  *  Health Examination Form (for entering Pre-School, Kindergarten, 3rd and 6th Graders) MUST BE COMPLETED AND MAILED TO THE SCHOOL OFFICE BY JULY 15th.
    *  2010-2011 Financial Commitment Form

** If you choose to complete your “Registration” form on-line (see #1 above), you will need to have a “Screen Name” and “Password” for FastDirect. All St. Joseph Parents have a screen name and password.
